VIRTUAL PROGRAM REGISTRATION

A separate registration is required for each registrant. Registration is not complete until fee is paid.

FEE SCHEDULE

$500: Physician

$350: Reduced fee:

  • Physician-in-training, with verification of status provided on employer or program letterhead. (Send verification to UIC Otolaryngology by email or fax. See contact information below.)
  • Non-physician
  • UIC or Mayo otolaryngology alumnus

PAYMENT

The following credit cards are accepted for payment: MasterCard, Visa, Discover, American Express.

CANCELLATION/REFUND POLICY

Email notification to UIC Otolaryngology is required to cancel a registration. Direct message to: Mariela Garcia, marielag@uic.edu.

Refunds will be subject to a deduction of $50 for cancellation notifications received by February 10, 2021. No refund will be issued for cancellation after that date.
