MEETING REGISTRATION & DISCOUNTED LIFT TICKETS

Meeting Registration

The Registration Deadline (for best rates) is February 10, 2017. A separate registration must be completed for each registrant. See below for cancellation and related refund information.

The Symposium Registration Fee — $850 ($950 after February 10) — includes a full breakfast each day and afternoon refreshments. A reduced fee of $550 ($600 after February 10) is available to physicians-in-training (with letter verifying status), non-physicians, and registrant spouses who are not otolaryngologists.

Physicians-in-training: Verification of status, on employer or program letterhead, should be sent to the UIC Department of Otolaryngology–Head and Neck Surgery by email attachment, fax, or mail. (See "Contact Us" below for address and fax number.)

Discount Lift Tickets

Pay later (including on site), or pay by check, to avoid a 5% transaction fee applicable to credit card refunds. In order to pass along the best possible discounts to you, it is essential that the meeting avoid incurring charges for processing credit card refunds associated with lift ticket purchases. See below for instructions on how to reserve tickets and create a balance due. You are welcome to pay for your lift ticket order onsite.

Please plan to reserve your lift tickets in advance, by February 10, as possible, to ensure that the necessary inventory will be available. To place or adjust a lift ticket order, after you register: Your registration confirmation will include a confirmation/ID number, which you can use to access your record and make additions or changes, under “Merchandise." Contact us if you have a problem locating this number, or need other personal assistance.

Exceptional Group Discounts available through the symposium — for registrants, as well as accompanying family and friends:

Adult Tickets:

Each of our adult 4-, 5-, 6-, and 7-day tickets includes a free day of skiing.

Why we don’t offer 3-day adult tickets: Since the price of a 4-day ticket includes a free day, we would need to charge the same price for a 3-day ticket, if offered.

Why the 4-day ticket provides the lowest cost per ski day: One out of four days is free, vs. one out of 5, 6, or 7 days for higher denomination tickets.

Child, Teen, Senior Tickets:

Child (ages 7–12), teen (ages 13–17), and senior (ages 65–69) tickets are priced the same ($58/day for multi-day tickets!). Children 6 and under ski free, only the $5 charge for radio frequency card applies (these tickets need to be purchased at a lift ticket window). Over 69? — contact us for information on additional ticket options.

Our adult lift tickets entitle you to savings on ski and snowboard lessons, as well as purchases or rentals at Aspen Skiing Company retail outlets, including ski and snowboard shops and participating on-mountain restaurants. These offers will come loaded onto your ticket and are only available onsite. You will need to show your lift ticket to qualify for these savings.

Payment Options

Reminder regarding lift ticket purchases: It is recommended that you pay later (including on site), or pay by check, to avoid the 5% transaction fee that will apply to credit card refunds associated with lift tickets.

Your registration is not complete until your registration fee has been received but, as noted, you are welcome to reserve lift tickets and pay for those later, including on site. To pay less than the total amount shown on your account, simply enter the amount you wish to pay and that is what you will be charged at check-out.

Be sure to make your registration fee payment by February 10 (2017) to avoid a late fee. Lift ticket rates will be the same, regardless of when you make your purchase.

Payments may be made by credit card (American Express, MasterCard, Visa, Discover) or check. Checks are payable to "University of Illinois" and should be mailed with copy of confirmation to the UIC Department of Otolaryngology–Head and Neck Surgery. (See "Contact Us" below for address.).
